Cancellation Policy:
By scheduling, you agree to observe the a 24hr cancellation policy and agree to pay in full for any missed appointments or appointments cancelled within 24hrs of appointment unless related to a Covid-19 health concern.

If you reply "yes" to any of the following questions, please contact us immediately 
to reschedule your appointment without penalty.
1. Have you had a fever in the last 24 hours of 100°F or above?
2. Do you now, or have you recently had, any respiratory or flu symptoms, sore throat, or shortness of breath?
3. Do you now, or have you recently had, any chills, muscle aches, new loss of taste or smell, or new rashes
or lesions?
4. Have you been in contact with anyone in the last 10 days who has been diagnosed with COVID-19 or has Coronavirus-type symptoms?
 
Masks are optional in the office but
